Course Overview

Accurate recording of vital signs is fundamental to patient safety and clinical decision-making. This practical course teaches you how to measure and record temperature, pulse, respirations, blood pressure, oxygen saturation (SpO2), and capillary blood glucose correctly and safely.

Clinical observations (vital signs) are the first line of defence in detecting clinical deterioration. Understanding normal values, recognizing abnormalities, and knowing when to escalate concerns are critical skills for all healthcare staff.

Through demonstrations and extensive hands-on practice, you'll develop competence and confidence in performing these essential clinical skills accurately and consistently.

Theory Session (60-90 minutes)

Note: Participants will practice on each other (peer practice) for some observations under supervision. Blood glucose testing is practiced on training devices and willing volunteers only.

Clinical Skills Covered

Temperature Measurement

  • Oral, tympanic (ear), and axillary (armpit) sites
  • When to use each method
  • Normal ranges: 36.5-37.5ยฐC
  • Recognizing fever and hypothermia

Pulse Assessment

  • Radial, brachial, carotid, femoral sites
  • Rate (60-100 bpm normal adult)
  • Rhythm (regular vs irregular)
  • Strength and character

Blood Pressure

  • Manual measurement technique (auscultation)
  • Automated devices and their limitations
  • Correct cuff size selection
  • Normal range: 90/60 to 140/90 mmHg
  • Hypertension and hypotension recognition

Respiration Rate

  • Counting breaths accurately
  • Normal rate: 12-20 breaths per minute
  • Respiratory pattern assessment
  • Signs of respiratory distress

Oxygen Saturation (SpO2)

  • Pulse oximeter application
  • Normal values: 95-100%
  • Factors affecting accuracy
  • When readings may be unreliable

Blood Glucose Monitoring

  • Capillary blood sampling technique
  • Glucometer use and calibration
  • Normal range: 4-7 mmol/L (fasting)
  • Hypoglycemia and hyperglycemia recognition
  • Sharps safety and disposal

  • Peer Practice: Participants will practice taking observations on each other under supervision
  • Clothing: Wear clothing that allows access to arms for blood pressure practice
  • Workplace Competency: After training, supervised workplace practice is required before independent recording
  • Group Size: Maximum 12 participants to ensure adequate hands-on practice
  • Refresher: Skills should be updated annually and competency reassessed

Assessment & Certification

Assessment includes:

  • Knowledge check covering theory and normal ranges
  • Practical demonstration of each vital sign measurement
  • Accurate recording on observation charts
  • Identification of abnormal findings

On successful completion, participants receive a Safe2Care Clinical Observations certificate. Workplace competency assessment is required before independent practice.
